About This Clinic

The clinic provides general dental care for youth, ages one through 18, whose families are low income and have no health insurance. The clinic primarily services people who live in the Burbank area, however, there are no geographic restrictions.

The clinic provides general dentistry, cleanings, fillings and x-rays. To be eligible for service, families must fall within the Federal low-income guidelines. Services are provided by volunteer dentists and dental hygienists and assistants.


APPLICATION PROCEDURE

Call to schedule an appointment. Patients must bring picture ID and Social Security Number for each child. Proof of income includes two years of IRS 1040 tax returns with W-2s and the past 8 weeks of pay stubs or a letter from employer.


FEES/PAYMENT SOURCE

The fixed fee is $15 per visit, per child
