UCFS can assist you in meeting your financial obligation in the following ways:
Sliding fee scale: UCFS offers a sliding fee program for clients who are uninsured or underinsured and whose income is below 200% of the federal poverty level. UCFS’ sliding fee scale is based upon a client’s family size and household income. Clients who do not provide income information are responsible for services at UCFS’ standard rate. Patients must provide the following information when applying for a sliding fee scale:
Two (2) most recent pay stubs from employer, unemployment and or alimony checks.
A letter from employer, on company letterhead, disclosing income (if no pay stubs available).
Proof of Social Security income.
