No Insurance
Optimus Health Care offers a Sliding Fee Program Discount based on income and family size, for those patients who do not have insurance. You will be asked to complete an application and provide our office with income verification (two recent pay stubs), address verification (utility bill) and identification. You must be prepared to pay a sliding fee which will be determined at the time of visit.
