Who qualifies?
The Dental Clinic in Crestview provides services to children ages 4 to 18 and who are on Medicaid.
We accept Integral, Wellcare/Staywell, Sunshine, MCNA, Liberty Dental, Dental Health and Wellness and United Health Care.
Effective August 1, 2015 the Dental Clinic will no longer accept Humana or DentaQuest for dental services.
The clinic also accepts Argus and DentaQuest Healthy Kids plans.
We recognize the need for children's dental services. The goal of our dental clinic (810 E James Lee Blvd, Crestview) is to provide oral health services to the underserved population in Okaloosa County. Our dental team consists of dentists, dental assistants, and other professionally trained staff.

Healthy Tips for Parents
• Start early! ...as soon as the first tooth appears!
• Use fluoride toothpaste for children ages 2-6. Children under 2 should not use fluoride toothpaste unless advised by a doctor or dentist. Use an amount the size of a pea.
• Supervise. Watch them brush twice a day.
• Talk to your dentist!
