This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
The clinic serves children and adults who are at or below the 200% Federal Poverty Level and have no other access to health care. Services include primary health care and some specialty care.
Hours: 1:00pm-2:30pm, Mon, Tue, Thur (No appointment necessary). Clinic medical services by appointment only.
Intake Process:
Individuals must come to the clinic on registration days and bring the following documents:
* Picture ID
* Proof of Address
* Proof of family Income for the last four weeks.
Eligibility:
* Children and adults must live in Miami-Dade County.
* Must be at or below the 200% Federal Poverty Level.
* Must not have any type of health care insurance or coverage.
* Cannot have Medicaid or any other federal or state healthcare coverage.
