This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
Scheduling a visit:
New and existing patients should call 773.233.4100/8524. We accept Medicaid, most insurance plans, credit cards, cash, as well as a sliding fee scale for patients who qualify based on proof of income.

Mon-Wed 8 am – 6 pm
Thurs 11 am - 7 pm
Fri 8 am - 5 pm
Sat 8 am – 1 pm (every 1st and 3rd Sat)
