Focuses on preventive exams (in children, starting at age one or the first tooth). Also provides fillings, cleanings, and emergency treatment for children.
Fees: Discounts are based upon family size and income. Proof of income is required to be considered for a discount. Discount levels are 25%, 50%, 75%, and 100% on eligible services. There is a minimum $30 fee due per dental visit, regardless of discount due at the time of service. Discount eligibility is re-evaluated annually or sooner if a person's financial situation changes. Crusader accepts Illinois Health Connect, Medicaid, Medicare, and many private insurance plans.
