About This Clinic

Dental equipment includes an electronic dental record, modern digital x-rays, and a flat screen TV in front of every dental chair to display films about your procedure and dental health. Focuses on preventive exams (in children, starting at age one or the first tooth). Also provides fillings, cleanings, and emergency treatment for children.


Fees: Discounts are based upon family size and income. Proof of income is required to be considered for a discount. Discount levels are 25%, 50%, 75%, and 100% on eligible services. There is a minimum $30 fee due per dental visit, regardless of discount due at the time of service. Discount eligibility is re-evaluated annually or sooner if a person's financial situation changes. Crusader accepts Illinois Health Connect, Medicaid, Medicare, and many private insurance plans.
