A recent paycheck stub (within the last two weeks) on yourself and everyone in your household who works.
If you are not working, bring proof that you are seeking employment.
Current income tax return and W2s.
Current Medicaid denial letter.
Clinic Hours
Monday-Thursday, 9am – 3pm and Friday 9am-11am to screen persons for clinic services.
Tuesdays and Thursdays, at 5pm for medical clinics and pharmacy services. Patients must make appointments prior to coming to the clinics.
