About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Offer a sliding scale fee for self-pay patients, and accept Medicaid and Medicare, along with most insurances.

To qualify for their sliding fee scale, you must fill out an application with proof of all household income, and this must be submitted at your initial exam appointment.  

Sliding fee scale benefits are typically determined within 1 week and someone from the clinic will contact you to schedule your next dental appointment.  

A letter will be sent to you in the mail with proof of your approval within 1-2 weeks. 

Once your application has been processed, your benefits will be determined. 

If you do not have any insurance, they can set you up with their patient navigator to see if you qualify for NH Medicaid benefits.
