About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

 MVCHS provides a full range of dental services including but not limited to:

   

  • X-rays
  •     Exams
  •     Cleanings
  •     Fluoride treatments
  •     Fillings
  •     Extractions
  •     Sealants
  •     Crowns
  •     Dentures
  •     Space maintainers
  •     Head Start Exams

Fees for services will be assessed utilizing a Sliding Fee Schedule that considers family income and number of family dependents.

When other sources of payment are available to the registered patient, the sliding fee schedule will be considered the last source of payment.

Eligibility for the sliding fee schedule must be updated annually to ensure eligibility and consider changes in income and/or number of dependents. Registration can be changed or updated more often if financial status changes.

Financial counseling which may include MOSAA eligibility determination, sliding fee determination, payment arrangements, social service referrals and other efforts to support patients in maximizing their resources will be available to all patients with need.
