It is Middletown Community Health Center, Inc.'s mission to provide
quality healthcare to all members of the community. As such, MCHC
participates with most insurance plans and offers a sliding fee to
uninsured patients.
Uninsured/ Self Pay Patients
MCHC offers a sliding fee scale based on incomes up to
200% of the Federal Poverty Guidelines and family size. This is
available for patients who do not qualify for health insurance. Proof
of income is required at the time of the visit to determine eligibility
for the sliding fee scale.
