Sliding Fee Scale: Self-pay patients are eligible for a sliding fee scale. To determine eligibility for the sliding fee, the following documents will be accepted: check stub, W2 forms, SSI, Social Security benefits, VA benefits, etc. Payment is due at the time of service.
