About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

This is not a FREE clinic. It is a sliding fee scale clinic.

Health Partners of Western Ohio is an independent, non-profit, community-directed organization serving low income and medically underserved communities.

They will not turn anyone away for inability to pay.

Health Partners accepts Medicaid, Medicaid Managed-Care (Buckeye, Paramount, Molina, CareSource, etc.), Medicare, and most commercial insurances. Individuals who have no insurance and an income that falls below 200% of the Federal Poverty Guidelines can receive a discount (Sliding Fee Scale) on services. Health Partners also has Financial Counselors available to help complete Medicaid and Health Insurance Exchange (ObamaCare) applications.

Ask to speak with a Financial Counselor. They are available to explain the Sliding Fee Scale and assist patients in completing a screening application. In order to receive a discount, patients are required to provide proof of income.
