Our eligibility staff can determine if you or your family qualifies for Medicaid, CHIP or other local, state and federal programs. So that we can best assist you, we ask that you bring the following information to your first appointment:
Proof of income for everyone in household. This can be recent pay stubs or tax return statements.
Confirmation letter for any other income (SNAP, SSI, disability, VA benefits, etc.)
Proof of residency. This can be a recent utility bill, lease or county tax receipt.

Across the United States, free and low-cost dental clinics play a vital role in making oral healthcare reachable for everyone. Supported by government funding, non-profit organisations, and charitable contributions, these clinics deliver essential dental treatment to those who need it most — including uninsured individuals, low-income families, children, seniors, and people receiving government assistance.
