This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Community Health Centers, Inc. is unique in Salt Lake County. It is their mission to provide comprehensive, top-quality services for the underserved, including both comprehensive medical and dental care.

CHC offers a sliding fee schedule to discount the cost of medical or dental care for those patients with incomes below 200 percent of the federal poverty line. All eligible patients who apply for this discount program must provide proof of income. Patients are expected to pay appropriate co-pays at time of service.
