CHAS Health offers a Sliding Fee Discount Program that can reduce the cost of your care.
To apply for the Sliding Fee Discount Program:
1. Meet with a CHAS Patient Services Coordinator (PSC).
2. Fill out a CHAS Sliding Fee Discount Program Application with the PSC.
3. Provide proof of your income:
Pay stubs, recent tax return, unemployment statements, SSI letter;
Child support statements, letter from employer, L&I award letter, bank statement;
or, Signed Self-Declaration
