About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

The Clay-Battelle Health Services Association offers a sliding fee
discount program to help with out of pocket expenses to all patients
based on your household size and income. This discount program allows
you and your family to pay a reduced fee for covered medical and dental
services.

In order to qualify for our sliding fee scale, you will need to complete a sliding fee application and provide proof of income. Acceptable proof of income would include
your tax return, W-2, recent pay stubs, Social Security benefits
statement, or other documentation
