About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Capstone Rural Health Center provides a Sliding Fee Discount Program for all patients that are uninsured or underinsured and whose total household gross income falls at or below 200% of the federal poverty guidelines. The sliding fee discount is used to calculate the basic discount and is updated each year using the federal poverty guidelines.
The sliding fee discount applies only to services provided by Capstone providers.


Adult Medical Services:
Capstone has two full time Nurse Practitioners that are always accepting established patients, new patients, and walk-ins. They accept all insurances and have no limit on Medicaid or Medicare services. They also accept the uninsured patients on a sliding fee program. They offer a wide range of primary and preventative health care services at affordable rates including but not limited to:
 Routine primary care, including diagnosis and treatment of illnesses
 School, work, sport and DOT physical exams
 Chronic Disease Management
 Women’s health, including PAP smears and mammograms
 Men’s Health, including colon and prostate screenings
 Immunizations
 Sick Visits
