Eligibility and Cost of Services
Florida Department of Health in Levy County strives to make services available to as many people as possible based on our available resources.  Eligibility and or fees for services vary by program.
We accept BlueCross and Blue Shield, Medicare, Avmed and Medicaid Managed Care Plans including Staywell, Prestige, and Sunshine.  We accept the following Medicaid Managed Care Plans for children:  MCNA, Liberty, Argus, United and Dentaquest.

Hours: 8:00am - 5:00pm Monday through Friday
Dental Services for children and extractions only for adults
Services Provided
Diagnostic Services: Exams and XRays
Preventative Services: Cleanings, flouride treatments and sealants
Restorative Services: Resin-based fillings, stainless steel crowns and space maintainers
Basic Oral Surgery: Extractions
