About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Dental Services for Kids:


Emphasis on children and early prevention of cavities
Dental health exams, treatment and education, fillings, cleanings
Digital dental x-rays
Electronic medical/dental records


Children walk-in dental emergency appointments: 8 am – 3 pm daily


Fees:

Discounts are based upon family size and income.

Proof of income is required to be considered for a discount.

Discount levels are 25%, 50%, 75%, and 100% on eligible services.

There is a minimum $30 fee due per dental visit, regardless of discount due at the time of service.

Discount eligibility is re-evaluated annually or sooner if a person's financial situation changes.


Insurance:

Crusader accepts Illinois Health Connect, Medicaid, Medicare, and many private insurance plans.
