Provides a sliding fee discount program to those who qualify. To qualify for the sliding discount, you must fill out a form (these can be picked up at any of the offices) and bring proof of income into any of our offices. A minimum payment of $30 per Medical visit and 50% per Dental visit, is expected at time of services. While they are not a free clinic, they are able to provide reduced costs to those who qualify while providing quality health care to all.
